Is there a form that I can use to file a motion to compel?

Not really. Just say that you sent the following interogatory or request to produce, the time to respond has passed (or there was an objection) and then a BRIEF explanation about why the requested information is relevant or likely to lead to the discovery of relevant information. It is hard to give you more helpful information without additional facts.

Check local rules . . . . you may need (for instance) to attache the actual question. Most courts also require you to attempt in good faith to resolve the substance of the motion by discussion with the opposing party and your motion should state that you have done that but could not resolve the issue.
