Where to purchase legal forms?

I need to file a lein against an estate of a person that has past on. It is for care given that was never paid to me.

I also need to file a lawsuit against an estate of which my brother n law shot me and is now deceased. His estate is open and I had a lawyer who decided to drop the case because the worth of the estate was not worth his while. I cannot afford an attorney and would like to do this on my own.
