Type of contract needed?
I am going to be utilizing a Dr. to develop a tool/survey for me. I have a contract for the intial work/development covered. However, I am going to keep him on board to answer questions from participants/misc. in return I will give him 2 shares of my LLC. I need to know what kind of contract I would need to accomplish this.

you need to see a lawyer. This really can't be answered on over the internet. These can be quite complicated.

There is no name or form for every type of agreement. You will need to hire an attorney, explain your objectives, and have the attorney draft an agreement for you incorporating those objectives. One size generally only fits all in men's socks and even then, not perfectly.
