waiver

i need to find a form to print out. i am releasing an employee for working here (firing) and i need some form stating that i am not responsible for his work (AS A MECHANIC) IF A CUSTOMER WAS TO COME BACK WITH PROBLEMS

There is no such form. As the employer, if your employee negligently repairs a car, then you are responsibel for his negligent -- whether you fire him or not. A release by him cannot affect your liability to the customer.
