Is it possible for a greencard approved 3 years ago to be revoked for any reason?
Does it really happen in practice?

Yes, it is possible. If you have spent much of the time outside the US, the government could determine that you relinquished permanent residence (PR). Your PR could also be revoked if there was fraud in the original application, or if you did something to cause you to be removed (deported) from the US. This can happen even after a person becomes a PR.
