Legal Question in Immigration Law in Florida

I am Peruvian and I came here with tourist visa b1/b2, I got married with my bf 2 months ago and we sent all the papers to make my residence. We received a letter telling that we need to send more evidence like my birth certificated in English and my I94. But also they attached the Supplement of status under section 245, Do i need to fill that form? cause the questions are kind of weird. Thanks
