Legal Question in Insurance Law in Florida
A patient has a consult with a doctor and provides all the requested insurance information, the doctor performs surgery ten days later, (doctor had 10 days to verify coverage.) then when payment is claimed against the patients insurance payment is denied, the doctor finds out they don't take the patients insurance. Who is responsible for payment?

The patient. You signed forms that state that you are responsible if insurtance does not pay for some reason. It is the patient's responsibility to verify coverage, not the doctor's.
