Not reporting an appointment to an insurance form on an application form
When we filled out our insurance application, my wife reported she had knee surgery on two seperate occasions. She didn't tell them she had an appointment and received advice about her knee, even though she didn't receive any treatment or surgery. My questions: (1) will we lose our insurance if they found out about this (2) would my wife face a legal problem (fines, jail, etc.) because of this ommission.

The answer is no to both questions. You listed her prior medical condition. If they were concerned they would required a physical exam.
