If house is not to code (roof leaks, ceiling has fallen in areas and evident mold) and the tenant is legaly disabled - does landlord have to pay to move disabled? The landlord has been notified since May 2011 and has not fixed and says to move if you do not like it.

Although your facts are light. Assuming that the property is not habitable in the current condition and tenant must seek a new location, the landlord could be liable for moving costs, 3 months rent and damages related to the move and to property. I would suggest you have code enforcement come out and inspect the property.
Note that if the property is untenable, the city or municipality may condemn the premises and you would have to move out immediately. Govern yourself accordingly.
