Rhonda, I am so sorry, but without looking at your forms, I do not know the answer. My best guess is the \"Agent\" is the attorney-in-fact to whom you must present the form to affirmatively revoke his power of attorney. But, if you need to present the forms to others to revoke his power of attorney, I cannot know without more detail and without seeing the forms. \n\nHave you looked to a legal aid clinic for help? There are legal clinics in FL, and I bet they could help you with this.
