Hello:
It sure sounds like you went through a scary episode. First, insurance companies try to save money by suggesting that you are partly (20%) to blame for a car collission. Notice I did not say accident, because I doubt the other driver accidentally crossed 3 lanes.
I practice in New Jersey, Philadelphia and South Florida.
Second, you certainly have a valid claim for legal damages including the costs of future surgery. An accident victim only has one shot to be compensated for collissions.
I'd be happy to talk to you. You may call my cell at 215.850.9400.

NOTE: This communication is not intended as and should not be interpreted as legal advice. Rather, it is intended solely as a general discussion of legal principles. You should not rely on or take action based on this communication without first presenting ALL relevant details to a competent attorney in your jurisdiction and then receiving the attorney's individualized advice for you. By reading the "Response" to your question or comment, you agree that the opinion expressed is not intended to, nor does it, create any attorney-client relationship, nor does it constitute legal advice to any person reviewing such information, nor will it be considered an attorney-client privileged communication. If you do not agree, then stop right here, and do not read any further.
Whatever you do, do not sign anything before reviewing this matter with an attorney. Once you take any monies, you may waive your rights to any future claims. Insurance adjustors are famous for trying to give accident victims much less than what they deserve. They will tell innocent victims to stay away from attorneys and say nasty things about the profession. You must remember, however, that your attorney is there to protect you. He or she works for you and not the insurance company. The only person who is truly on your side in this situation is the one you call your attorney.
From your fact pattern, it seems as if you do have a good case. You need to schedule an appointement with an attorney of your choosing who you feel will diligently represent you and your interests. Good luck.
