Adding name to deed

What is the proceedure ,and what forms are required to add a spouse's name to an existing real estate deed as a joint tenancy with rights of survivorship?

Go to any legal stationary store or title company and ask for a blank quit claim deed. Type out the form with you as grantor to you and your spouse jointly as grantees. File deed with county clerk
