General affidavit in puerto rico

There is a general affidavit form signed by a notary for all states (it does not mention puerto rico) on the web. I need it so my mother in law can give up her rights to a cementery plot in Puerto rico. Can I use that form as long as I include the legal description of the plot and give it over to her sister?
