You might try an internet search for such a form, or contact an attorney with a copy of the recorded lien, and ask him to prepare one. It would be relatively simple and inexpensive. Your co-worker would then sign it before a notary and you would record it in the official records of your county.

Not all forms are available in an office supply store. Many legal documents have to be drafted by attorneys. You might find an example in a law library but you seem to not have had any luck.

For a nominal fee, an attorney will prepare a release of lien in the proper format to release the lien from your property. If it is not done properly, it will be ineffective and not serve your purpose. To guarantee the release is drafted correctly, you should retain an attorney to do this for you.
