Taking over a mortgage

Is there any way to start paying a mortgage that the owner can't pay anymore (with the owner consent) and have in writing the once the mortgage is paid off the property is now my property? or can we change the name on the deed?

1. You cannot change the deed without the lender's consent.

2. You can arrange for the transfer of title.
