what forms do i need to transfer my home from my name to my living trust??

You can effect the transfer to the trust by a Quit Claim Deed. I would suggest that you have this done by an attorney so that no errors are made which would invalidate the transfer and possibly cloud your title.

A Warranty Deed (preferable) or a Quit-Claim Deed from the owner(s) to the trustee(s) of the living trust. Recording the deed will require you to reapply for the homestead exemption and have a least a tempory effect on your real estate taxes (depends on how your county handles it). I strongly recommend you have a knowledgable attorney help you with this. If real estate gets messed up with an improper or incorrect deed, it can be a bear to fix.
