warranty deed on home

I'm divorced. The warranty deed still has my name and my former husband's name as husband wife.

We do still wish to have the other receive all equity in the home should one of us die. How can we revise the warranty deed to do this?

A new deed as joint tenants with right of survivorship can be filed.
