what format is used to submit a motion to change a plea in County Traffic Court.

Different judges use different forms. In traffic court, most judges don't even use forms for changes of plea from not guilty to guilty or no contest. You just tell the judge that's what you want to do. Some judges have a form called a "written plea of no contest or guilty." These forms are usually kept in the courtroom.
If you want to go the other way, from guilty or no contest to not guilty, the procedure is much more complicated. That requires a motion to set aside plea. You'll probably need a lawyer to help you with that.

Not sure what you mean by format. All pleadings should have the same case style.
