Yes. You will have to prove the original is missing.

Yes, Yes and it will require substantial proof as to why the original is missing. There is going to be presumption that the absence of an original will means it was destroyed by the owner and no longer effective. Hire counsel to discuss this issue in detail.

Yes - and you will need to locate the attesting witnesses - you will need one of them to sign an affidavit.
