What if I have no family or friends, and I'm not a member of any synogogue, but I do have a burial plot. Who can then carry out my wishes as no one will even know when I die. Can a lawyer do it? Are there people who can be hired (in advance) to do it? I don't want to end up in Potter's Field someday. I'm afraid that a Will will not be good enough as there would be no one to notify a lawyer of my death. I'm afraid that even if there was someone (such as a hospital) by the time someone at a hospital would contact the lawyer, the State might have done as they please with my remains. I'm asking because I'm 68 years old and not in good health, so I'm starting to wonder what will happen to me.

Yes you can appoint a third party to take action after your death. You can also pay for pre-need burial plans and they can be executed upon your death by the funeral home. Seek some legal guidance on creating that Estate plan.
You certainly can retain counsel to carry out your wishes. You should have a will and health care proxy. Good luck.
As a practical matter, I would also advise that you leave instructions in a place that can easily be found by anyone who might come in to your home to assist you. For example, in the event of emergency, contact "Lawyer". In the event of death, I have a burial plot at "Cemetery"
