which form and how do I access it, to put in place a living will for the state of florida

There are forms everywhere including on the internet and maybe even on the lawguru.com site. The person needs to sign the form in front of two witnesses who also sign and at least one of the witnesses must be someone other than a spouse or a blood relative. The form must also be notarized at that time. Give a copy to physicians/hospitals and one to next of kin.

I would suggest to anyone interested in a Living Will that they speak with an attorney to insure it is done correctly. A Living Will is only one of several estate planning documents everyone should have. For some general information take a look at Florida Statutes 765.301 through 765.309.
