my mother is in icu on medication she has just started talking and certain family members are trying to manipulate her into signing away her medical rights to them? is it legal and binding if she signs under the influence of medication

It sends up some serious red flags.
If you want to protect her, you may want to send in a psychiatrist to examine her and give a medical opinion on her mental state. If they try to do something, this doctor will be able to testify of her condition.
