What do I need to do in order to change the name in the surviving spouse item in my husband's death certificate?

I was still married to him by the time he passed away. Nevertheless, he married another woman while still was married to me. Please I need help.

You must contact the physician who signed the death certificate, explain the

issue, and request a change. You may discuss the matter with the Office of Vital

Statistics, Dept of Health, but they will advise you to contact the officials

responsible for the error or issue.
