Permission for Medical Care While Parents Are Out of Town

My husband and I will be in Hawaii on vacation for a week and our two children, 17 and 13 will be staying with their grandparents during that time. What do we need to do to give the grandparents permission to make medical decisions concerning the children in our absence?

I would suggest a simple signed and notarized power of attorney giving the grandparents authority to act for you in this area. I would include insurance information, current doctors, allergies and allergic reactions, and information on any current medications. You might contact you local hospital as some have such forms for people like you or contact a local attorney. Have fun on the trip. In addition, this might be a good time to review your will. More importantly, I trust you have taken care of any possible guardianship for the children should something happen to you. If that issue is not covered in your will, it should be. Authorization for Minors Medical Care While Parents Unavailable

FACTS: Parents going on vacation for a week; minor children staying with grandparents.

Q. What do we need to do to give the grandparents authorization to make medical decisions concerning the children in our absence?

A. Execute a written document giving them permission to act and have it notarized and witnessed. A Durable Family Power of Attorney would be a good start. Check with your local school and hospital for any forms they might have or call your attorney.
