Legal Question in Wills and Trusts in Florida

Real Property & Domestic Partnership

We have a certified domestic partnership from New York and now permanently reside in Florida. My name is the only one on the deed. This was planned on. Now, upon my incapacitation (stroke,etc.), I want my partner to own it totally without me. How is it done? Trust? Will? POA?

Add there name to a new deed with you as joint tenants with right of surviviorship.
