Setting Up a Trust in order to Pay Nursing Home

I need to find out how to set up a trust automatically paying a nursing home for my elderly father. Can I use generic forms and then go to a bank or must I use an attorney? If I cannot, can you recommend a local attorney at minimal costs? The nursing home is having me go through the state to see if he will qualify for Medicaid, but I've been through that twice now and he's been denied both times. I would like to have all my ducks in a row by month end and know what my options are. Thank you.

FACTS: Nursing home is having me go through the state to see if my father will qualify for Medicaid. Like to have ducks in a row and know my options.

A. Recently someone contacted me with this same situation for their Mother. The Nursing home gave them information on a Social Security income trust. They type up what they wanted and sent it to me for review. I made some changes and sent it back. I haven't heard from them and assume it worked. It cost them one hour of my time. Your question was posted without an address so I can't suggest a Local Atty. as I don't know where you live.
