where can i get a small claims affidavit for my bank
and also i need a letter of adminstration

You obtain letters of administration through Florida probate proceedings. The letters of administration is a document signed by the probate judge. The letters of administration gives the person appointed as personal representative authority to collect and manage assets of the person that died.
