What this document does, in its standard form, is tell the probate you (1) do not want to be personal representative, (2) you want the person named in the form to be personal representative, (3) you will allow the person named to be appointed personal representative without the court holding a formal hearing and giving you notice of that hearing, and (4) you agree that the personal representative can serve in that capacity wihout obtaining an insurance bond to protect heirs from any bad acts he/she might commit while personal representative.
