what kind of deed has to filled out to covery joint tenancy and rights of survivorship

Any type of Deed can create the joint tenancy. The type of Deed (Warranty, Bargain and Sale or Quit Claim) only goes to the quality of title being conveyed, not how the property is to be titled. I suggest using either a Warranty Deed or Bargain and Sale.
