if injured on the job who is responsible for paying my health ins.

No one really. What happens is that if the employer does not continue the health insurance, the carrier has to include in your lost wages two thirds of of the amount THE EMPLOYER paid into the insurance. The insurance does not have to be continued and you may not be able to get an amount in your lost wage check that is anywhere close to what health insurance may cost you. You may be eligible for Cobra benefits but you have to pay for that. Sorry for an answer you probably don't want to hear.
