My workers comp carrier request a Functional Capacity test is it mandatory that I attend ?

Probably. If your treating doctor made a request for it, then if you do not attend the carrier could cut-off your benefits by claiming that you failed to cooperate. Talk to your doctor and see if he really thinks it is needed or if he can assign permanent restrictions without it. Otherwise you should attend and do your best and ask questions if you are not sure how to do each test to ensure a valid result.
