do I have to let the workers compensation rep come into the exam room when I go to the doctor

No. And do not sign anything that this person ask u to. Inform the doctor or staff that you do not want her in the room and that you revoke any prior signature. If they won't cooperate with you, find a qualified attorney who specializes In Workers comp to file a motion for protection with the court.
