need to find a form
I need to find a form to submit to the court saying I wish to represent myself. My laywer withdrew from my divorce case. I need to let the courts know of my intentions.

If there is no attorney in the case it is assumed that you represent yourself but the document you are referring to is an "entry of appearence". The clerk of court could provide you with the appropriate way to write it.
