Clearing of chapter 13 off credit report

I filled for chapter 13 in 1994 and have fully paid off all of my debtors and discharged by the court. It's been 12 years and I still have it on my credit report. How many years does it stay in the credit report? What can I do to remove this entry?

Instructions for disputing an entry should be on the report, or in a document included with the report. You can also call the bureaus for instructions.
