I'd STRONGLY suggest you get an attorney to do this for you. The cost is minimal compared with the damage you cause if you get it wrong.
If you had an attorney and it was his mistake, call him. If you made the expensive mistake of not having a lawyer, get one now, so he can also review your case for any other possible fixes.
