Where can I get a hold harmless statement and recommended insurance requirements for patrons using my nightclub to hold events?

You are going to face lawsuits and you are not going to be protected by internet forms. You need a lawyer to draft a document personalized to your facts. Even with those, you will still face suits and huge legal defense costs, win or lose. Since one drunk patron leaving your club and killing a few people in a crash could be a $10 or $50 million suit, you want at least $50 to $100 milion in liability coverage, and far more than just waiver forms. You want policy documents and protocols for bouncers and bartenders and servers, and have a taxi company on call.
