This is more of a consumer problem than a legal one. Everyone makes mistakes and giving them ONE chance is fine. But three second chances? Frankly that's not okay.
You can't sue someone for almost dying. But you can sue for what you paid for the repair. Make a demand for a refund, in writing, by certified mail and give them 5 days. If they refuse, make a complaint with the Better Business Bureau and the Governor's Office of Consumer Affairs. If you get no result within 30-45 days, sue the business in Magistrates Court for what you paid (plus court costs), using their small claims forms.
And before you take the car elsewhere for repairs, research the business with the Better Business Bureau and online rating sites.
