What forms do I need to file in order to sue a Debt Collection agency for non-compliance?

You file a complaint, which is not a "form" but a document that sets forth specific facts as required under the law and rules. A lawyer will be able to draft this (and serve it, and properly prosecute it) -- assuming you have a valid complaint. You can also try magistrate court, if your county has one.
