unauthorized debit
A major financial institution attempted ''two'' unauthorized debits on my account. The second one after I had informed them of the first error. I have no ties to this company what so ever, and never have.

You don't actually ask a question, and you leave off many details. Did they credit your account after the first time? Have you provided notice after the second incident? What was their response?
