Distributing a Release

I will be compiling a book of short blurbs on the types of secrets that women keep from men (in the vein of the ''Chicken Soup'' series). This is an independent project totally funded by me. Do I need people to sign a release before I accept their submissions (via the web) so that I can use their short blurb? If so, what type of release should I have and what kind of information should be included in the release? Also, since I will have people submit their blurbs electronically via a web site, would it be sufficient to have the individual click a checkbox (an electronic signature of sorts) stating ''I agree to this release'' or something similar, or would this be something that requires an actual written signature?

I would not recommend a "check-the-box" release for something like this. They are often not completely upheld due to the nature of the consent. There is no form for the release. You should speak with a local business attorney who will be able to ascertain your specific needs for the release.
