What form to use?

I am working on a book that would include healing stories from individuals. I need a release form allowing me to publish the information that was provided from the individuals.

See a qualified lawyer to draft the appropriate documents. There is not some generic "form" that covers everything and even if there was, it would be a poor idea to use with without a review.
