Amending Birth Certificate/Name Change

Mother and father were not married at the time child was conceived although they married a month after child�s birth. Father wants to sign birth certificate. Mother and child both agree. Child is currently over the age of 18. What procedures must be followed in order to change child last name?

Correcting a birth certificate and changing the last name are two separate causes of action. An attorney with some expertise can likely save you money and work those into one case. Few attorneys have done the correction, and it involves serving the state with a superior court action. An adult name change is more routine.
