amending birth certificates

I need to know how to remove a father's name from a birth certificate and replace it with the biological father's name. The child has already been legitimized and had her last name changed to her biological father's last name. I need proper filing procedures and forms if available. Thanks!

Take a certified copy of the final order of legitimation to the office of vital records and pay the required fee to make the changes on the birth certificate. The order will serve as direction to the department of vital records to amend the birth certificate.
