Birth Certificate Changes

My daughter is 14 years old. At the time of her birth I was not married, but was, at time of conseption to her father. On the birth certificate (under father) it says information denied, because he was not there. He now wants his name on her birth certificate. So what legal forms (if any) would we file to have his name placed on the birth certificate and her last name changed to his?

This requires a court case in Superior Court (there is no "form" and each case must be drafted with exact specifics or Vital Records will ignore the order, so you need an experienced lawyer who has dealt with this before). My office handles such cases, so if you're in metro Atlanta, call me.
