Name change on birth certificate

The father of my daughter refused to sign her birth certificate. It simply has ''refused'' in the space for the fathers name. Her father has not been seen for 6 years and I have been married for 5 years. My husband wants to put his name on her birth certificate and give her his last name. She calls him Daddy. What steps do we need to take to have this happen?

Step one is hire an adoption attorney. I do them regularly so feel free to call me to discuss details.

The case will be easiest if the father will sign a consent. It can likely be done without his consent but that will increase the cost and leave more possibilities for complications as his rights then must be terminated.
