name change

I am divorced with full custody of my child. Now I am wanting her name changed to my maiden name. I am not sure which forms to use however, I was informed this is a possibity.

It's possible, but this is not as simple as getting your maiden name back. You will have to file a petition to change her name in the superior court, publish a notice in the paper, and serve your ex. There will be a hearing and the Judge will listen to all objectors. If there are no objections or issues he will grant your petition. You didn't give the age of the child, that could be an issue, or the reason she wants it changed.
