name change of minor

i have spoken with the court clerk, and she seems to be able to tell me nothing about this. i have sent the father of my child 2 seperate name change petition forms for him to sign and return to me, and he hasnt returned them. he hasnt seen our daughter since april of this year, and hasnt payed child support since july. i know that i am supposed to have his permission to change her name, but i have an email from him stating that he isnt going to see her anymore, and he has stopped sending payment since i asked him to change her last name. i dont want her to have to grow up carrying a complete strangers lastname. is there anything else i can do about this? thanks in advance.

need his consent. However, you can claim that he has abandoned the child and then you will not need his consent.
