Name change for 1 year old

My daughter has her fathers last name upon his insistence. He also signed her birth certificate and a PA form. Unfortunately, he has decided not to be involved in her life. How do I go about getting her last name changed to mine?

You hire a lawyer to seek a name change in Superior Coiurt. With his consent it will be rather simple. Without his consent it is up to the court. My office handles these. Call me if we can help you.
