where can i find forms to change custody of my 16 year old daughter without having to go through an attorney

To my knowledge, there are no such forms. We attorneys draft the documents on a case-by-case basis, depending on the facts and circumstances of each case. Even in areas of the law where there are forms, these so-called "one size fits all" forms, if not properly tailored to the particular case, can give a false sense of security. Get a lawyer.
