Can I refuse to let my ex get my son for visitation unless he gives me his physical address?

Yes, if your agreement/decree so provides. Otherwise, no.

It depends on what the decree says. If it isn't addressed, then the answer is probably not. However, you should consult with a local attorney to review the decree as well as seeking a modification to have the address issue added. It might be that your local judge would grant an emergency hearing, since common sense tells us that the mother has a right and a need to know where her child is.
